Important. Please Read:
Once you have chosen and booked your session, please read this and complete the form in good time before the session. It is important that you do this for three reasons:
1. It gives you a chance to organise your thoughts, so that you can get the most out of your session. It doesn't have to be in the greatest detail, but every little helps. As a part of the process try to keep it to under 1000 words of normal narrative (no bullet points, or notes). It is a good focus for you and it will stop you wasting part of the session explaining basic stuff. This summary should include information on who you are as a person, if you have any serious mental, or emotional conditions and if you are receiving treatment from a health care professional. Background information on work, your family, friends and hobbies is all useful. Some information about why you have chosen to take the session will be helpful as well. At this stage it is not necessary to formulate specific goals and ambitions. The amount of personal information you give is up to you. As this is a confidential and potentially anonymous service, the only information that is really crucial is your age (over 18) and any medical conditions that you have, that might affect your health during sessions.
2. It means that we hit the ground running. I will have a working idea of who you are and what you need. I will be able to arrange any handouts and audio recordings that he think will be helpful to your session.
3. We will be unable to start our session until you have read the therapy agreement (asked any questions you might have by email) and completed the tick box on this form. Don't worry it is not a huge document full of legalese, but it is important for both parties that we understand the nature and extent of the therapeutic agreement.
